Investigation Summary
At 6:25 on February 8, 2008, Employee #1 was working for a maritime cargo handling firm. He entered a hatch to a cargo hold and was descending a fixed ladder to the next lower level. He had climbed down approximately 10 feet, when he fell backward from the ladder about 50 feet to the lower level. He struck at least one side platform during the fall. After Employee #1 was removed from the hole, he was pronounced dead at 7:01.
