Investigation Summary
On January 17, 2008, Employee #1, an experienced equipment operator, was adjusting the front fork tines on a forklift. The retaining pin to hold the front fork assembly on the equipment had not been positioned properly, causing it to fall and strike Employee #1 on the back. He was hospitalized for his injuries.
