Investigation Summary

Investigation Nr: 201955317
Event: 10/16/2007
Employee Is Killed When Run over by Truck

On October 16, 2007, Employee #1, a truck driver, and a coworker left their base in Michigan with loads bound for Lowe's Distribution Center in Washington Court House, Ohio. Employee #1 arrived 15 minutes ahead of his coworker and pulled into the staging area. He began removing the straps, which held the tarps in place over his load on the flatbed truck. Employee #1 was standing on the ground, center of the bed of his truck, continuing to unstrap his load, when his coworker arrived. Employee #1 motioned for his coworker to pull into the staging area, parallel to his truck. As the coworker pulled in, witnesses observed Employee #1 slip and fall to the ground. He tried unsuccessfully to get out of the way before being killed by the outer rear tire of the second truck.
