Investigation Summary
At approximately 4:30 a.m. on May 8, 2008, Employee #1, a forging shop foreman, was walking next to the left rear tire of an industrial lift truck. The truck made a 180 degree right turn and struck Employee #1 in his leg, which became entangled in the rear wheel. Employee #1 was hospitalized and his leg was amputated.
