Investigation Summary
On August 30, 2007, Employee #1, a new machine operator and adhesive sprayer in his second week of employment, was spraying flammable contact adhesive onto tires inside the spray area of a 10,000 square foot production building. Employee #1 typically sprayed five tires at a time as they hung and spun on a tire spinner machine. Employee #1 had just finished spraying adhesive onto the tires, when he pressed the tire spinner machine electrical button, located in a control box within the spray area. He suffered second- and third-degree burns to sixty percent of his body in an explosion and fire. No other employees witnessed the accident. However, they did report hearing a loud "pop." Emergency Medical Services stated that Employee #1 said he saw a spark. Flammable contact adhesive vapors were in the area, and an explosion and fire occurred. Employee #1 was taken to the Jaycees Burn Center in Chapel Hill to be treated.
