Employee Falls from Ladder and Later Dies

At approximately 4:00 p.m. on September 3, 2008, Employee #1 was climbing one of two ladders used to access the roof over the garage area. As he neared the roof, the unsecured ladder slipped away from the edge and he fell approximately 7 to 8 ft onto the concrete driveway. Employee #1 suffered head and shoulder injuries and was transported to a local hospital by an Offutt Air Force Base ambulance. Employee #1 died thirteen days later on September 16, 2008. The 16-ft aluminum extension ladder was unsecured and according to a witness did not extend to the necessary three feet past the access area.
