Investigation Summary
On March 31, 2008, Employee #1 was welding steel I-beams together when his clothing caught fire. Employee #1 was taken to a nearby medical facility, where he was hospitalized and treated for third-degree burns, covering over 30 percent of his body. The accident investigation revealed that Employee #1 was not wearing the proper personal protective equipment that should have included leathers.
