Investigation Summary
At approximately 6:20 am on November 10, 2007, Employee # 1 and Employee #2 were hooking up their truck-tractors to empty trailers at dock door. Employee #1 was in the truck and was going to pull the truck-tractor and trailer forward so that Employee #2 who was at the back of the trailer could look in the trailer to make sure it was empty. Unfortunately, Employee #1 had failed to lock in the fifth wheel kingpin into the hitch. When the truck-tractor and trailer started to move forward the trailer kingpin came out of the hitch and the trailer rolled back pinning employee # 2 between the trailer and the dock. Employee # 1 then backed the truck- tractor up to lock the fifth wheel kingpin into the hitch and was able to pull the trailer away from the dock and unpin Employee # 2. Employee #2 was pronounced dead at the scene.
