Investigation Summary
On October 12, 2007, an employee and several coworkers were loading a forklift onto a tandem axle flat bed trailer. The trailer was not hitched to a tow vehicle. The trailer tipped up and the employee fell off. Coworkers stated that the employee landed on his head. The employee died in the hospital two days later.
