Investigation Summary
On October 7, 2008, an employee was working near the neighboring business whose worker was welding components of a catwalk over a waste oil storage tank. The welding activity ignited waste oil flammable vapors that resulted in the storage tank exploding. The explosion force threw the employee resulting in unspecified physical and psychological injuries. The employee was hospitalized.
