Employee Is Killed When Struck by Mortar Mixer

At approximately 7:24 a.m. on June 11, 2009, an employee, a foreman of the mason crew, was mixing a batch of mortar when the mortar mixer inadvertently tipped forward and pinned the employee up against a mortar tub that was positioned in front of the mixer. The employee was killed. The mortar tub was hanging from the forks of the rough terrain forklift. The mortar mixers tires and axle had been removed and the mixer had been placed on a base of sand at the time the accident occurred. At the time of the accident the employee was working alone in an area approximately quarter of a mile from the other 3 employees working for the company.
