Event: 11/26/2008
Employee Is Injured by Forklift, Later Dies

At approximately 10:00 a.m. on November 26, 2008, Employee #1 and a coworker were conducting maintenance activities on a Toyota forklift (Model Number 7FGU30). Employee #1 and the coworker were working on the mast of the forklift to dislodge a chain that had jammed between a pin and the forks. The coworker was assisting Employee #1, who was lying below the forks and trying to get the chain loose by hitting the pin with a hammer and a pneumatic chisel. Employee #1 used a 4-inch by 4-inch nominal piece of wood to brace the fork, and no other bracing or blocking was used. When the chain broke loose from the pin, the forks fell approximately 22 inches, onto Employee #1's head and legs. The coworker assisting Employee #1 and a second coworker lifted the forks off of Employee #1 and pulled him out from underneath the forks. Emergency medical services were called, and Employee #1 was transported via helicopter to the hospital, where he later died.
