Investigation Summary
On November 4, 2008 at approximately 12:45pm, Employee #1 fell off an open 8 foot fiberglass step ladder inside of the Silgan Containers plant in Oconomowoc, WI. At the time of the accident Employee # 1 was working by himself and was wiping down walls and preparing the walls for painting. Right before the accident Employee #2 walked by Employee #1 to throw something into the garbage, they exchanged some small talk, and after Employee #2 threw away his garbage he turned around to return to work and noticed Employee #1 wrapped up in the ladder and on the ground. Employee #2 then called for help at which time coworkers responded and 911 was called. At the time of the accident the work area appeared to be clean and dry and the ladder also appeared to be in good condition. A call from the Waukesha Co. Medical Examiner stated that Employee #1 died from head trauma and that it appeared no health problems contributed to the accident.
