Investigation Summary

Investigation Nr: 201250263
Event: 10/26/2009
One Employee Is Killed; Two Are Injured by Falling Covers

Employees #1 and #2, cemetery employees, were moving and cutting marble and granite covers. The covers were six feet by eight feet and weighed approximately 300 pounds. There were approximately 12 covers. The covers were leaning on a metal stand similar to a horse scaffold, which was fastened to the floor. There were approximately six covers on at each side of the stand. The covers extended beyond the side of the stand and only a few inches over the top so the covers from one side did not touch the covers from the other side. While Employees #1 and #2 were working on one cover, the stand failed and the covers fell on both employees, killing Employee #1 and injuring Employee #2 and Employee #3, the owner
