Investigation Summary
On July 21, 2007, Employee #1 was spraying off debris from the spikes of the head machine in the kill room while the machine was running. If workers need to remove debris by hand that the sprayer cannot remove, they must lock out the machine first. Employee #1 was found by a floor supervisor, yelling for help while the spikes were detaching his right arm and puncturing his chest area. Employee #1 was hospitalized.
