Investigation Summary
On July 24, 2007, Employee #1 was coming out of the restroom when he was struck by a forklift. The forklift (Model Number 2EC25-E; Serial Number A2EC363023) was being driven forward with no load and with the forks near floor level when the accident occurred. The forklift struck the employee and drove 4 ft, with the employee's lower left leg stuck under the underbody of the forklift. The employee was transported to the Shock Trauma Center in Baltimore, where his left leg was amputated above the knee. Employee #1 was hospitalized. The accident investigation found that while the forklift operator was trained in the safe operation of forklifts, he did not sound the horn of the forklift when driving through the area. There were pallets of goods that were stacked approximately 10 to 12 ft tall along the wall of the restrooms, which obstructed the view of employee pedestrians and forklift operators. Convex mirrors were not provided to enable forklift operators to see other forklifts, and/or pedestrians, and vise-versa. It was also noted that permanent aisles for forklift traffic were not marked.
