Investigation Summary
On August 29, 2007, Employee #1 was cutting a 2-in. by 4-in. piece of wood on a 20-in. chop saw. He reached his hand into the guarded cage of the chop saw to retrieve a piece of wood while the blade was running. He contacted the blade with his right hand, amputating four fingers and was hospitalized.
