Investigation Summary
Employee #1 was installing water proofing to foundations of new single family homes under construction. Employee #1 pulled the hose out from the truck and kept it slack to enable movement of the hose during spraying. A coworker controlled the nozzle of the spray hose. Employee #1 climbed onto the foundation wall, and as he pulled on the hose, he fell from the wall, causing injury to his back. He was an employee of Morais Enterprises, Incorporated
