Investigation Summary
At approximately 10:00 a.m. on June 10, 2008, Employee #1 was working from a ladder approximately 12-ft to 15-ft above ground level. Employee #1 was setting trusses. Employee #1 was landing a truss using a hammer, held with both hands, to guide and position the truss. Employee #1's hammer lost contact with the truss and his momentum carried him backwards off the ladder onto the concrete. Employee #1 suffered bruises and abrasions and was hospitalized.
