Employee Is Pinned Under Standup Fork Truck

At approximately 8:45 a.m. on October 23, 2007, Employee #1, a fork lift operator, was unloading a 53 ft trailer at a distribution warehouse using a Crown standup lift truck. An independent contractor truck driver received his completed paperwork from the receiving office. The truck driver thought he heard the dock plate drop, signifying that the unload was complete. The driver removed the wheel chock and began to move the vehicle forward to close the rear swing doors. The movement of the trailer away from the dock caused Employee #1 to be thrown from the lift truck into a sitting position on the dock plate. She was pinned by the upper mast and overhead guard of the lift truck. She was hospitalized with severe internal injuries and permanent leg damage.
