Employee Is Killed When Struck in Head by Small Log

At approximately 1:30 p.m. on August 17, 2007, Employee #1, the head supervisor of the crew conducting tree trimming operations, was towing an aerial lift. The aerial lift weighed approximately 10,700 pounds, and became stuck in soft terrain while operating in the front yard of a residence. Employee #1 took it upon himself to free the stuck aerial lift. Employee #1 selected a 0.625-in. synthetic rope and looped it through one of two steel eyelets located at the rear of the aerial lift. After threading the rope through the eyelet Employee #1 looped the rope tying a knot and then inserting a small log. The other end of the rope was then secured to a split truck that was positioned approximately 100 ft from the aerial lift. The crew had succeeded in freeing the aerial lift pulling it for approximately 20 ft when the rope broke violently retracting back towards the tow or split truck. Employee #1 who positioned himself near the split truck was initially struck by the rope and eventually by the log that he previously attached to the rope. The log struck Employee #1in the head killing him instantly.
