Investigation Summary
On or about September 14, 2007 at approximately 4:30 p.m., an employee was removing a jammed plastic milk jug from a trimming machine. The employee raised the plastic guard, and believing he had activated the interlock, placed his right hand between the knives of the trimmer to remove the jug. The machine continued to operate crushing and cutting his right hand. He left his work station to seek medical attention. His supervisor summoned emergency medical services and the employee was transported to OSHU.
