Employee Suffers Unspecified Fracture in Fall from Ladder

On November 21, 2007, Employee #1 was restocking and moving boxes on a four-tier steel upright shelving unit. Employee #1 was using a mobile ladder stand, while standing on the second or third step from the top of the platform. The locking mechanism was not working or was not engaged. The platform move forward, causing Employee #1 to lose his balance and fall approximately 4-ft to 5-ft onto an epoxy concrete floor. Employee #1 sustained unspecified fractures and was hospitalized.
