Investigation Summary

Investigation Nr: 202526844
Event: 10/11/2007
Employee injured in fall when wall overturns

At 11:56 a.m. on October 11, 2007, Employee #1 was working for Pacific Coast Steel at a site in Stanford CA. He was between 15 and 20 ft up on a shear wall of rebar steel, disconnecting a rigging from the crane, when the wall started to fall to one side. Employee #1 was wearing fall protection and held on as the shear wall came down. He sustained head trauma, with lacerations on the left side of his head. Emergency units were called and the Palo Alto Fire Department arrived and transported Employee #1 to Stanford Hospital. Pipe bracing was in place on the shear wall, but only secured on one side; the wall fell to the opposite side of the bracing. The employer had asked the crane operator if he was cleared before he moved away from the shear wall and he responded that he was clear.
