Investigation Summary
On August 7, 2007, Employee #1 of U-Pull-It Auto Parts, Inc., was riding on the right running board of an Omega rough terrain forklift. Employee #2, the forklift driver, was making a left hand turn in front of the drain rack shed. Employee #1 stepped off or fell off the running board of the forklift and was immediately struck by the right rear wheel of the vehicle. Employee #2 felt a bump and stopped the vehicle. Employee #3 was riding on the left side of the vehicle. There were no witnesses to the accident. Employee #2 was looking toward the left and the other passengers' view was blocked by the body of the vehicle when Employee #1 fell from the vehicle. Upon dismounting the vehicle, Employee #2 found Employee #1 lying behind the forklift. Employee #2 was unable to feel a pulse and attempted CPR. Employee #1 was pronounced dead at the scene of the accident by the authorities.
