Employee Is Killed in Fall from the Top of Fuel Tank

On June 12, 2007, Employee #1, a heavy equipment mechanic, was working with a coworker replacing a hydraulic line on the rear driver's side of a 1996 Gradall XL4100 in Bay Number 18. After replacing the hydraulic line, the coworker climbed onto a rolling ladder and got on top of the equipment, where he began filling the hydraulic fuel tank with approximately 100 gallons of fluid. Employee #1 climbed the rolling ladder and got on top of the engine platforms, which was 7.5 feet above the concrete floor, and set a cover plate in position at the rear driver's side of the equipment. Employee #1 and the coworker were discussing what size bolts would be needed to finish installing the cover, when Employee #1 fell off the edge of the truck and down to the concrete floor. He fell on his face and head, killing him.
