Employee Is Killed by Falling Wall

At approximately 8:32 a.m. on March 29, 2008, Employee #1, a foreman, was moving a prefabricated wall with supports on both sides with the help of two coworkers. The plan was to move the prefabricated wall with the use of a forklift; however, a forklift operator was not available. The employer provided a site safety program: however, it did not specify the safest method to move a prefabricated wall. The wall fell on Employee #1, who was killed.
