Investigation Summary
At approximately 3:20 p.m. on December 6, 2007, Employee #1 was planting aquatic plants with a coworker inside a lake. When he walked, he suddenly sank into the marsh and drowned. The interview with the coworker revealed that Employee #1 could not swim. At the time of the accident, Employee #1 was not wearing personnel protective equipment, based on the decision made by the supervisors, who thought that the employees were not exposed to the drowning hazard. The company records did not show that Employee #1 was trained in the recognition of drowning hazard. The company had a Safety Program in place but the program did not specifically address the drowning hazard. There were no contributing weather variables the day of the accident.
