Employee Is Electrocuted and Killed

At approximately 10:30 p.m. on November 27, 2007, Employee #1, a manager, was replacing a defective gearbox with a new "David Brown" gearbox (Model Number A16021208RN-13). In order to replace the gearbox, he had to remove the "Baldor Industrial" 208-230 volt electric motor, (Serial Number W0702052181). The motor supplied power to the "mule", a high horse powered gearbox with low rpm. He had asked a coworker in the vicinity of the start/stop station to turn the power on, so that he could test the gearbox. However, when he completed the task, he did not cover the junction box located underneath the electric motor because he was looking for a filler cap that was missing from the gearbox. From a crouched position, he went to stand up and started to lose his balance. He grabbed the eyebolt on top of the "mule" with his left hand and reached over with his right hand and grabbed the uncovered junction box that had exposed wires that were energized. He was electrocuted when he grabbed the eyebolt of the "mule". Employee #1 was killed. The employee had performed electrical work before, but he was not a certified electrician.
