Employee dies of head injuries after fall from ladder

On October 23, 2007, Employee #1 was working from a ladder, preparing to remove some plywood from a mansard roof. The job foreman was working within 20 ft of him, but not was not within visual contact when he heard the ladder fall. Employee #1 fell around 6 ft and was found lying on top of the ladder. He suffered severe head injuries and was transported to a trauma center, where he was declared brain dead. On October 25, 2007, Employee #1 was removed from life support and died.
