Employee dies after fall from lift basket

At approximately 1:15 p.m. on June 19, 2007, Employee #1 and a coworker were using an aerial lift truck to remove a 15 ft tall cast iron street lamp from the ground. The lamp post was in a grassy area on the sidewalk and the coworker had dug around the base to loosen it. Employee #1 then placed a nylon sling with a slip knot around the neck of the post. He was starting to lift the post out of the ground when the sling slid off the top of the post and the bucket bounced. Employee #1 was ejected from the basketand fell approximately 15 ft to the ground. He suffered severe head trauma and was placed on life support at the hospital. Employee #1 died the following day.
