Employee Amputates Finger While Boring Out Flange

At approximately 9:10 a.m. on February 3, 2010, Employee #1, of Benicia Fabrication & Machine Incorporated, was boring out a 90 in. flange using a Betts vertical turntable/lathe with attached carbide tipped cutting tool. While boring the flange, a large chip of the flange became stuck on the flange and would not fall away. At this point, Employee #1 reached in with a pair of needle nose pliers to remove the large chip. His left fourth finger tip became caught between the needle nose pliers and the tool/cutting bit, which caused the 4th finger tip to be amputated. Employee #1 was transported to the hospital for treatment.
