Investigation Summary
Employee #1 was cleaning plaster debris with a supervisor on the first level of a steel tube scaffold on the northwest side of Building Number 29. Employee #1 climbed to the second level and while attempting to climb to the third level of the scaffold using the top rails and vertical sections of the scaffold, Employee #1 slipped and fell 16 ft onto a concrete curb and asphalt road. Employee #1 sustained multiple bruises in his ribs, left leg, neck and back. Employee #1 was hospitalized.
