Investigation Summary
At approximately 1:45 p.m. on January 26, 2009, Employee #1 was working as part of an inmate crew splitting wood on the Mt. Diablo grade project. While stacking split wood approximately 10 ft from where the wood was being split with a wedge, metal fragments became embedded in his eye. He was hospitalized for treatment. Employee #1 was wearing eye protection at the time of the accident.
