Employee Amputates Finger While Operating Tommy Gate

At approximately 1:17 p.m. on October 7, 2008, an employee returned to the yard with a broken demo hammer. The employee went and got a track loader to remove the equipment from the rear of his truck. The employee used a Caterpillar track loader 12066-60-69, Model Number 247, and positioned it at the rear of his truck. A coworker came over to assist and opened the tommy gate at the rear of the truck. The employee requested that the coworker close the gate but the coworker was called away and did not close the gate. The employee already had positioned the arms of the track loader up in the air. The employee was not able to reach the piece of equipment with the gate of the truck open with his track loader. The employee had a partial amputation of his right middle finger when he reached out with his right hand from his track loader to close the gate. The employee was hospitalized and treated for finger amputation.
