Employee Lacerates and Fractures Hand in Vacuum Extractor

At approximately 12:30 p.m. on October 6, 2008, an employee was operating a TTE Engineering laser welding machine in Line Number 4 with no visible model or serial number. The machine jammed where the pipe pieces are extracted from the conveyor. The employee went inside the perimeter cage guard of the machine to clear the jammed area of the conveyor. The main interlock guard of the machine was bypassed to allow entry into the area while the machine was operating. The employee was attempting to remove a pipe; the axis which is located at the exit point of the conveyor moved to the down position resulting in a very deep, severe laceration to the right hand of the employee. The employee was hospitalized for five days for his injuries.
