Investigation Summary
At approximately 11:30 a.m. on October 2, 2007, Employee #1 was tieing rebar for the foundation of a petroleum refinery storage tank at a Valero Refining Company construction site in Benicia, CA, when he lost consciousness and collapsed. Two coworkers pulled him to a nearby container and kept him in the shade until an ambulance arrived. Employee #1 was transported to Sutter Solano Medical Center in Vallejo, CA, where he was admitted for treatment. It was later determined that he was suffering from serious heat exhaustion, despite taking numerous breaks in the shade that morning to drink water.
