Employee Amputates Finger While Cleaning Machine

At approximately 12:15 a.m. on July 26, 2007, Employee #1 was cleaning out pieces of a blue plastic shovel that had become crushed inside a dry ice forming machine. The machine had been turned off while the employees cleaned out visible pieces of plastic. The machine was restarted when all visible pieces were removed and Employee #1 supervised the operation of the equipment. After the machine produced a few more blocks of dry ice, Employee #1 noticed that blue plastic shovel pieces were inside the blocks and coming out of the machine. Employee #1 reached into the machine to further clean out the plastic pieces without deactivating and locking out the equipment. The machine activated, causing Employee #1 to suffer amputation of his left middle finger. Employee #1 was hospitalized.
