Employee killed in fall from roof work platform

At approximately 10:00 a.m. on August 9, 2007, Employee #1 was on a 2 ft by 12 ft working platform, installing a hip board to the ridge board of a residential home under construction. Employee #1 had mis-nailed the hip board, and while he was removing it to reposition it, he lost his balance and fell approximately 26 ft onto the concrete floor. He died of head injuries sustained in the fall. The work platform did not have guardrails and Employee #1 was not using any form of fall protection.
