Investigation Summary
On September 28, 2008, Employee #1, an executive chef, was with the coowner renting two Ford E-350 vans. Employee #1 drove one van out of the parking space, approximately 15 ft and stopped. He opened his door and leaned out to speak to the coowner, who was on foot looking for the second van in the yard. Employee #1's van started to roll backwards. According to a witness, Employee #1 was standing on the floorboard of the van and was partly leaning out of the van. According to the coowner, Employee #1 was running backwards on the ground and the driver's side door struck and knocked him down. When the van began to move in reverse, Employee #1 attempting to step on the brakes may have stepped on the accelerator pedal instead. The van jerked, and he lost his balance and fell flat on his back. Both witnesses saw Employee #1 runover by the 5,400 pound van. Employee #1 died at the hospital.
