Investigation Summary
At approximately 8:30 p.m. on June 26, 2008, Employee #1 and two coworkers were cleaning and servicing a Nutrition Services Dishwasher machine at a hospital. They failed to take positive means to deenergize or disconnect the dishwasher from its source of power by using lock and tag on the controls or switch box. While cleaning the outside of the machine, one of the coworkers accidently depressed the On-off Button. The machine discharged water from the upper jets with the access door in the open position. The hot water, approximately 160 degrees Fahrenheit, was released, striking Employee #1 in shoulder and neck. Employee #1 was hospitalized with burns.
