Employee Is Killed by Falling Load of Pipe

On December 4, 2007, Employee #1 was helping to load a flatbed trailer with PVC pipe. The load had been stacked two pallets wide and two pallets high. A strap had been thrown over the load to secure it to the trailer. Before Employee #1 was able to secure the pipe, another pallet of 10-in. PVC pipe was stacked on top. This load caught the unsecured strap on the corner of the pallet, leaving slack in the line. The load shifted and fell, striking and killing Employee #1.
