Investigation Summary
On August 2, 2007, Employees #1 through #3 were working on a 28 to 30 ft tall wall attached to a diagonal cable. The cable was attached to a plumb-up hook and ran from the first column on the north end of the wall to the third column on the other end. Employee #1 had attached his fall protection to a wire rope sling that was part of the cable system. The cable system severed from end to end and Employee #1 fell between 28 and 30 ft to the ground. Employees #2 and #3 also sustained injuries when the cable system failed. All three workers were transported to the hospital emergency room, where Employee #1 died. Employees #2 and #3 were admitted for treatment.
