Investigation Summary
At 9:15 a.m. on July 18, 2007, Accent Awnings Company arrived at the Whitney Ranch Center in Henderson, Nevada, to remove five metal awnings attached to the east exterior wall over various tenant locations along the outdoor strip mall. An eight man crew was there to take down the first and largest awning, which was secured to the building's exterior by 40 bolts. The bolts were anchored to stud beams inside the wall of the building. The awning measured 24-feet 7.5-inches in length, 43.5-inches in height, and 42-inches in width. The weight of the awning was unknown. The bottom edge of the awning was approximately 10 feet from the ground. The awning structure was framed with 1.25-inch diameter steel tubing and covered with three layers of 16-gauge galvanized steel. The removal process progressed to where the awning was suspended by a single bolt in the middle. This enabled the foreman to tilt the left end of the awning downward to rest atop ladder number one, which had been erected to stand approximately seven feet tall. The right end of the awning was to be supported and lowered by six men standing two apiece on three other ladders. As the left end of the awning was being pulled down to rest atop the far left ladder, the awning broke free from the wall, pulling the last bolt from its position, and falling to the ground. Significant injuries were sustained by Employee #1 when the awning struck him on the head, causing large lacerations and shoulder injuries. Employee #2 was also hit by the falling awning, causing him to receive bruises to the left side of his abdominal area and consequent neck pain.
