Investigation Summary
On June 18, 2007, Employee #1, an apprentice electrician, and coworkers were running string through conduit for phone lines as part of a renovation on a single-story professional building in Henderson, NV. Employee #1 had just finished tying string on the ends of five or six runs of fish tape that a coworker was feeding and pushing through the other end of the conduit. Employee #1 then proceeded to feed and push fish tape through the opening of an electrical box where the conduit for the phone lines terminated. He was using a Husky 12 ft portable trestle ladder, model 01-41200-00, with a maximum standing level of 9 ft 5-3/4 in., to access the electrical box, which was located on the wall about 16 ft from the ground. Coworkers heard Employee #1 scream and one turned in time to see him and the ladder fall. Employee #1's back and head struck the concrete floor as he landed and he suffered a closed head fracture and a subdural hemotoma. He was transported by Emergency Services to Sunrise Hospital, where he was placed on life support until he died at 8:37 p.m. on June 19, 2007.
