Investigation Summary
Employees were involved in deck forming operations on the Omega roller press machine. The sheet metal was not properly feeding into the cutter. The sheet was backed up to try and realign. Employee #1 entered into a restricted machine area to try and align the sheet while it was fed into the rollers on its way into the cutter. A coworker was working on the jog switch to feed the sheet. When the coworker working the switch jogged the rollers to feed the sheet, Employee #1's right hand was carried into the form rollers resulting in amputation of three fingers. Employee #1 was hospitalized.
