Employee Killed in Fall Through Ceiling

At 4:00 p.m. on April 15, 2007, Employee #1 accessed the roof of a church to get onto a catwalk, approximately 40 feet above the sanctuary floor, to replace a light fixture. While attempting to change the lighting towards front of the sanctuary, he fell through the ceiling tiles to the sanctuary floor, and was killed. At the time of the accident there were other employees in the sanctuary who observed him falling, but did not observe the events leading to the accident.
