Employee Killed in Fall from Roof

On May 3, 2007, Employee #1 was tarring a roof with four coworkers. The workers were working approximately 27 feet above ground level and were using a hoist to raise and lower asphalt and other materials to the roof. The hoist weighed about 100 pounds and was located approximately 7 inches away from the edge of the roof. It was anchored by several 90-pound bags of rolled roofing materials. Employee #1 moved the hoist to tar the area under the equipment. However, when the hoist was repositioned the anchoring weights were not replaced to stabilize the hoist. When Employee #1 started lowering the bucket to the ground, the hoist fell over the edge of the roof, pulling him with it. He landed on his face on the concrete and was killed.
