Employee Is Caught Between Spotter Trucks and Is Killed

On March 20, 2009, Employee #1 was assisting in performing inspections and evaluations in the repair and refurbishing of several off-highway yard spotter trucks (powered industrial trucks) in the exterior parking lot of a sales/service/repair and fleet management dealership for powered industrial truck and material handling equipment. Employee #1 started one of the yard spotter trucks (Vehicle #1), then exited the operator's cab and proceeded to a kneeling position on the ground behind Vehicle #1, facing the undercarriage and rear bumper. Vehicle #1 rolled backwards about forty-two inches, pinning Employee #1 between the rear bumper of Vehicle #1, and the front bumper of another yard spotter truck (Vehicle #2) that was parked directly behind Vehicle #1. Apparently, the air emergency brakes of Vehicle #1 were not engaged and/or the transmission malfunctioned while the vehicle was left running and unattended. Employee #1 was killed.
