Employee Is Killed When Struck in the Head

On August 12, 2008, a five man crew was attempting to remove a 70-in. tall, 22-in. wide back-up power supply cabinet from its shipping pallet. The 1,919 pound cabinet was lifted with the forks of a powered industrial truck. The brackets that secured the cabinet to the pallet had not been completely removed and the pallet hung to the cabinet on one side. The cabinet was then lowered and it came to rest tilted on one of the brackets. The tilted cabinet tipped over and landed on Employee #1, who was assisting with the pallet removal standing along the side of the cabinet. Employee #1 later died as a result of his concussion.
